What is PAR-Q?

The Physical Activity Readiness Questionnaire (PAR-Q) is a standard health screening tool used by gyms to:

  • Identify members who should seek medical advice before starting exercise
  • Reduce the risk of injury or adverse health events
  • Demonstrate duty of care and reduce liability
  • Create a record of member health status

Configuring PAR-Q Questions

Customize your health screening questionnaire:

Adding/Editing Questions

  1. Navigate to Settings โ†’ Health Screening
  2. Click "Add Question" or edit an existing question
  3. Enter the question text (e.g., "Do you have chest pain during physical activity?")
  4. Choose question type:
    • Yes/No: Simple binary questions
    • Text: Free-form responses for additional details
  5. Mark as "Critical" if a "Yes" answer requires follow-up
  6. Save the question

Standard PAR-Q Questions

ClearGym includes the standard 7-question PAR-Q by default:

  1. Has your doctor ever said you have a heart condition?
  2. Do you feel pain in your chest when doing physical activity?
  3. In the past month, have you had chest pain when not doing physical activity?
  4. Do you lose your balance due to dizziness or lose consciousness?
  5. Do you have a bone or joint problem that could be made worse by exercise?
  6. Is your doctor currently prescribing medication for blood pressure or heart condition?
  7. Do you know of any other reason you should not do physical activity?

You can customize these or add additional questions specific to your gym.

Liability Waivers

Protect your business with legally binding digital waivers:

Setting Up Your Waiver

  1. Go to Settings โ†’ Health Screening
  2. Click "Edit Waiver Text"
  3. Enter your liability waiver content (or use the default template)
  4. Include essential elements:
    • Assumption of risk statement
    • Release of liability
    • Indemnification clause
    • Acknowledgment of gym rules
  5. Save the waiver

When Members Sign Waivers

Members complete the PAR-Q and waiver during signup:

  1. After selecting a membership plan
  2. Before payment is processed
  3. Member reads the waiver in full
  4. Member types their full name as electronic signature
  5. Timestamp and IP address recorded for legal validity

Reviewing Health Responses

Access encrypted member health information when needed:

Viewing Individual Responses

  1. Navigate to Members
  2. Click on a member's name
  3. Scroll to "Health Screening" section
  4. Click "View PAR-Q Responses"
  5. Responses are decrypted and displayed in real-time

Flagged Members

Members who answer "Yes" to critical questions are automatically flagged:

  • Red indicator displayed on member profile
  • Staff can filter to see all flagged members
  • You should contact flagged members to:
    • Discuss their health concerns
    • Request medical clearance if necessary
    • Create modified exercise plans

Viewing Waiver Signatures

Access digital waiver signatures:

  1. Go to member profile
  2. Click "View Waiver" in Health Screening section
  3. See:
    • Full waiver text they agreed to
    • Electronic signature (full name)
    • Date and time of signing
    • IP address (for legal verification)

Annual Medical Reminders

Keep health information up to date with automated annual reminders:

How Annual Reminders Work

  1. 12 months after initial PAR-Q completion, member receives an email
  2. Email prompts them to review and update their health information
  3. Member logs in and completes a fresh PAR-Q
  4. New responses replace old responses (previous responses archived)
  5. Process repeats annually

  1. Go to Settings โ†’ Health Screening
  2. Enable "Annual Health Reminders"
  3. Customize email template (optional)
  4. Set reminder frequency:
    • Single reminder: One email at 12 months
    • Multiple reminders: Follow-up emails if not completed

When necessary, you can manually delete health data:

  1. Go to member profile
  2. Scroll to Health Screening section
  3. Click "Delete Health Data"
  4. Confirm deletion (this is irreversible)
  5. Action is logged in audit trail
